Welcome. These Terms of Service ("Terms") govern your use of the Emergency Medical Form website and the free emergency medical form and magnetic packet ("the Service") provided by Financial Health ("we," "us," or "our"). By using this website or requesting a packet, you agree to these Terms.

Important: The Emergency Medical Form is a personal preparedness tool. It is not a substitute for professional medical care, and completing it does not guarantee any particular emergency response.

1. The Service

We provide a free emergency medical form and magnetic packet intended to help you keep important information available for first responders. The form and packet are provided at no cost. Optional donations are welcome but never required.

2. Eligibility

You must be at least 18 years old and provide a valid mailing address to request a packet. We reserve the right to limit quantities or decline requests to keep the program sustainable.

The Service does not provide medical advice, diagnosis, or treatment. Always seek the guidance of a qualified health provider with any questions about your health. In an emergency, call your local emergency number (such as 911) immediately.

5. No Guarantee of Outcome

We do not control how first responders act in any given situation and cannot guarantee that your form will be seen, read, or relied upon during an emergency. The Service is provided on an "as is" and "as available" basis.

6. Optional Donations

Donations are voluntary and help cover product and shipping costs. Unless otherwise stated, donations are non-refundable. Donations may not be tax-deductible; consult a tax professional regarding your situation.

7. Intellectual Property

The website content, design, logos, and materials are owned by or licensed to Financial Health and may not be copied or reused for commercial purposes without permission. You may use the form for your personal, non-commercial emergency-preparedness needs.

8. Limitation of Liability

To the fullest extent permitted by law, Financial Health and its sponsors, partners, and affiliates shall not be liable for any indirect, incidental, or consequential damages arising from your use of the Service or reliance on the form.

9. Privacy

Your use of the Service is also governed by our Privacy Policy, which describes how we handle your information.
